    Enrol With Us

    New Patients

    We welcome new patients to our practice. You may start the enrolment process here by downloading and completing the Enrolment form. Once complete please bring it to Little London with your NZ Birth Certificate or your passport and speak with our reception staff. You may also come in at any time to fill out the form in person, but please remember to bring an NZ Birth Cert or your passport with you as proof of your eligibility to enrol.
