Enrol With Us
As a family-centred practice, we value people and understand that visiting the doctor can be a stressful part of life, so we pride ourselves in making patients’ visits as comfortable and relaxed as possible with one-on-one contact the bedrock of our service.
There are 2 ways you can enrol:
Enrol online
Enrol in person
You may also come in at any time to fill out the form in person, but please remember to bring your NZ Birth Certificate or your passport with you as proof of your eligibility to enrol.
Please note that your driver's license is not a valid form of proof of eligibility as determined by the Ministry of Health.

If your proof of eligibility is not in your current name, please also bring documented evidence of your name change (e.g. a marriage certificate or Change of Name by Deed Poll).
